Service Provider Forms
(new set ups or updates):
For any provider not set up in our Hussmann OEM Warranty Claims system (software Tavant)- Corporate finance and legal require the proper set up forms.

Tax/Liability Information required:
- W-9 Tax form - Must be the most current IRS Form and signed within the last calendar year (US ONLY)
- NON-US Supplier Questionnaire - required so finance may advise which W8 form is required (NON-US ONLY)
- W-8 BEN - do not complete until Hussmann reviews your supplier questionnaire and confirms the proper form to return (NON-US ONLY)
- W-8 BEN-E - do not complete until Hussmann reviews your supplier questionnaire and confirms the proper form to return (NON-US ONLY)
Payment Information required:
- Remittance Statement Form (US ONLY)
- Wire Transfer Form (NON-US ONLY)
- Labor Rate Set up form - All providers are required to have an approved OEM Warranty Labor rate for Claims (ALL PROVIDERS)
User Information required:
- User Access for Provider Account Form (ALL PROVIDERS)
